UCPS - Create School Specific RISE Walkthrough Forms

1. Make a copy of the district form

1. Click "Forms"

2. Click "Add Form"

3. Click "Copy Existing Form"

4. Select "RISE Monitoring Walks - District Form"

5. In the form title: Replace "District Form" with the name of your school.

6. (Optional) Add a description to the form.

7. (Optional) Change the color of the form icon

8. Click "Copy Form"

2. Make edits to the form

9. To edit existing questions: click on the 6 dots to the left of the question. This opens a menu where you can choose to

  • make the question required

  • assign numerical point values to the options (for multiple choice with scores/rankings)

  • change the type (ex. from multiple choice to comment box)

  • link options (i.e. choose from other questions to create an association)

  • duplicate the question

  • delete the question

10. Click on the text to change the title, description, or selection options of the question

11. Use the 6 dots to drag and re-order the questions.

12. To add new questions: click on the plus icon next to any question to add a new question directly above or below the existing question.

13. Select the format of the new question (comment box, multiselect, etc.)

14. Click "Add to Form"

15. Give the new question a title, description (optional), or selection options (for multiple choice questions)

16. To add a new slide: click "Add section"

17. Give the new slide a title, and click "Add" to add elements (new questions, subsections, etc.)

18. To edit existing slides: click the 6 dots to the left of the slide to delete the slide. Use the six dots to drag and re-order the slides.

19. Click on the slide title or description to change the text.

3. Publish the form to your account

20. Click "Publish" when you are finished making changes (note: you can always edit the form later)

21. Click "Yes, publish"

4. Access your form

22. Click "Forms"

23. Click to open the form you made for your specific school

(notice: this form is not greyed out because you have access to edit it)

24. Click "Edit" to make additional changes.
